Thank you for your help streamlining the Monday morning check in process. We encourage you to return camp forms before the first day of camp through one of the following methods:
1. Camp forms can be submitted through our online registration system. After registering, you can log in to your online account to upload any required or optional forms at any time prior to your first session of Girlstart camp. Girlstart recommends this method for submitting all required and optional forms.
2. Email to camps@girlstart.org. Please include your daughter’s camp week(s) in the email copy or on the forms.
3. Mail in forms to:

Required forms include:
- Behavior Agreement
- Copy of current immunization record
If your daughter is attending multiple weeks of camp, the behavior agreement form and immunization records only need to be submitted once per summer by the first Monday of her first week of camp.
Optional forms include:
*Girlstart does not provide EpiPens, and campers that use EpiPens must bring their own EpiPen along with a signed medication release form the first day of their camp week. Additionally, Girlstart is mindful of allergies, however we are not a nut-free environment. Girlstart does provide alternate snacks to campers with allergies.
